Your first visit to Mundelein Pediatrics establishes a vital foundation for our relationship with you and your child. During the first visit, we make sure to obtain important background information, like your  child's medical history, pertinent family history, surgical history, allergies and immunizations. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Vaccine Policy

Mundelein Pediatrics is a strong advocate of disease prevention via vaccine administration. We implement a strict vaccine administration schedule; however, although it is not recommended, you may choose to do an alternate vaccine schedule as long as the child is fully vaccinated by the age of 2. If you choose an alternate schedule for your child, the child must have all vaccines recommended by the Center of Disease Control and American Academy of Pediatrics before the child turns 2 years old. Any patients over the age of 2, who have not been fully vaccinated yet, must agree to a catch-up schedule formulated by the doctor during your child's wellness visit. We will ask you to find another healthcare provider if you refuse to vaccinate or if you fail to adhere to the alternate/catch-up vaccine schedule.


What To Expect
Being well-prepared for your child's appointment will ensure that the doctor has all of the needed information to provide the best possible care for your child.  If you have a copy of any previous immunizations, please bring it along with you to the first appt. so we may record it in your child's chart. Also, please write down any medications your child is taking as well as the dosage. This will also help us get an accurate record and the ability to treat your child more effectively.  We look forward to your first visit!
